    目的 分析在黄褐斑患者中应用氨甲环酸联合1064 nm调Q Nd:YAG激光治疗对其皮肤症状及皮肤屏障功能的影响。方法 选取2024年5月-2025年2月无锡明德老年病医院皮肤科收治的80例黄褐斑患者,经随机数字表法分为对照组和观察组,各40例。对照组给予氨甲环酸治疗,观察组给予氨甲环酸联合1064 nm调Q Nd:YAG激光治疗,比较两组皮肤症状、皮肤屏障功能、临床疗效及不良反应发生情况。结果 观察组治疗后MASI、PGA评分均低于对照组(P<0.05);观察组治疗后皮损区表皮油脂含量、角质层含水量均高于对照组,经皮水分流失量低于对照组(P<0.05);观察组治疗总有效率(90.00%)高于对照组(65.00%)(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 氨甲环酸联合1064 nm调Q Nd:YAG激光治疗黄褐斑的效果良好,可有效改善患者的皮肤症状及皮肤屏障功能,且未增加不良反应发生风险。

    Abstract:

    Objective To analyze the effect of tranexamic acid combined with 1064 nm Q-switched Nd:YAG laser therapy on skin symptoms and skin barrier function in patients with melasma. Methods A total of 80 patients with melasma admitted to the Department of Dermatology, Wuxi Mingde Geriatric Hospital from May 2024 to February 2025 were selected, and they were divided into the control group and the observation group by the random number table method, with 40 patients in each group. The control group was treated with tranexamic acid, and the observation group was treated with tranexamic acid combined with 1064 nm Q-switched Nd:YAG laser. The skin symptoms, skin barrier function, clinical efffcacy and adverse reactions were compared between the two groups. Results After treatment, the scores of MASI and PGA in the observation group were lower than those in the control group (P<0.05). After treatment, the epidermal lipid content and stratum corneum hydration of lesion areas in the observation group were higher than those in the control group, and the transepidermal water loss was lower than that in the control group (P<0.05). The total effective rate of treatment in the observation group (90.00%) was higher than that in the control group (65.00%) (P<0.05). There was no statistically signiffcant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Tranexamic acid combined with 1064 nm Q-switched Nd:YAG laser achieves satisfactory efffcacy in the treatment of melasma. It can effectively improve patients’ skin symptoms and skin barrier function, without increasing the risk of adverse reactions.
