    目的 探讨断指再植皮肤缺损治疗中应用邻指指动脉岛状皮瓣修复术的效果及安全性。方法 纳入2022年5月至2025年6月我院收治的80例断指再植皮肤缺损患者,按随机数表法分两组,对照组40例行常规游离全厚皮片移植修复术,研究组40例行邻指指动脉岛状皮瓣修复术。对比两组手术康复指标、移植组织成活率,分别于术后3个月、术后6个月采用患者与观察者瘢痕评价量表(POSAS)、温哥华瘢痕量表(VSS)评估两组手指瘢痕外观修复质量,并记录两组术后并发症。结果 研究组创面愈合时间、住院时间显著短于对照组,移植组织成活率高于对照组(P<0.05)。术后3个月、6个月两组POSAS各分量表评分、VSS评分均随时间延长而下降,且研究组显著低于同期对照组(P<0.05)。研究组术后并发症总发生率显著低于对照组(P<0.05)。结论 邻指指动脉岛状皮瓣修复术可显著提升断指皮肤缺损修复成功率,缩短术后康复周期,美容效果好且并发症少,值得推广应用。

    Abstract:

    Objective To investigate the efficacy and safety of cross-finger digital artery island flap repair in the treatment of skin defects following finger replantation. Methods A total of 80 patients with skin defects after finger replantation admitted to our hospital from May 2022 to June 2025 were enrolled and randomly divided into two groups using a random number table, with 40 cases in each group. The control group underwent conventional free full-thickness skin graft repair, while the study group received cross-finger digital artery island flap repair. Surgical recovery indicators and graft survival rate were compared between the two groups. The Patient and Observer Scar Assessment Scale (POSAS) and the Vancouver Scar Scale (VSS) were applied to evaluate the quality of scar appearance at 3 and 6 months postoperatively. Postoperative complications in both groups were recorded. Results The wound healing time and length of hospital stay in the study group were significantly shorter than those in the control group, and the graft survival rate was higher (P<0.05). At 3 and 6 months after surgery, the POSAS subscale scores and VSS scores in both groups decreased over time, and the scores in the study group were significantly lower than those in the control group at the same time points (P<0.05). The total incidence of postoperative complications in the study group was significantly lower than that in the control group (P<0.05). Conclusion Cross-finger digital artery island flap repair can markedly enhance the success rate of skin defect reconstruction in finger replantation, shorten the postoperative recovery period, deliver favorable cosmetic outcomes, and result in fewer complications, thus warranting clinical promotion and application.
