    目的:分析负压引流(VSD)联合重组人酸性成纤维细胞生长因子(rh-aFGF)对难愈性创面的临床疗效以及对患者创面愈合指标的影响作用。方法:选取2023年7月—2025年7月在我院接受治疗的180例难愈性创面患者,使用随机数字表法分为观察组、对照A组和对照B组,每组60例。对照A组使用常规换药治疗,对照B组使用VSD治疗,观察组则使用VSD联合rh-aFGF治疗。比较三组创面愈合时间、创面愈合率、治疗总有效率;创面肉芽组织生长情况;创面微环境指标;不良反应率。结果:观察组创面愈合时间短于对照A组和对照B组(P<0.05);观察组创面愈合率高于对照A组和对照B组(P<0.05);观察组治疗总有效率高于对照A组和对照B组(P<0.05)。治疗后,观察组肉芽组织覆盖率、肉芽组织厚度高于对照A组和对照B组,创面pH值、渗出液IL-6、TNF-α水平低于对照A组和对照B组(P<0.05)。三组患者不良反应发生率对比(P>0.05)。结论:采用VSD联合rh-aFGF治疗难愈性创面患者,能够缩短其创面愈合时间,获取良好创面愈合率和治疗总有效率,促进肉芽组织生长,纠正创面微环境紊乱,且安全性良好。

    Abstract:

    Objective: To analyze the clinical effect of negative pressure drainage (VSD) combined with recombinant human acidic fibroblast growth factor (rh-aFGF) on refractory wounds and its influence on wound healing indexes of patients. Methods: 180 patients with refractory wounds treated in our hospital from July 2023 to July 2025 were randomly divided into observation group, control group A and control group B, with 60 patients in each group. The control group A was treated with routine dressing change, the control group B was treated with VSD, and the observation group was treated with VSD combined with rh-aFGF. The wound healing time, wound healing rate and total effective rate of the three groups were compared. The growth of granulation tissue on the wound surface; Microenvironment index of wound surface; Adverse reaction rate. Results: The wound healing time in observation group was shorter than that in control group A and control group B (P < 0.05). The wound healing rate of observation group was higher than that of control group A and control group B (P < 0.05). The total effective rate of observation group was higher than that of control group A and control group B (P < 0.05). After treatment, the granulation tissue coverage and granulation tissue thickness in the observation group were higher than those in the control group A and the control group B, while the wound pH value and the levels of IL-6 and TNF-α in exudate were lower than those in the control group A and the control group B (P < 0.05). The incidence of adverse reactions among the three groups was compared (P > 0.05). Conclusion: VSD combined with rh-aFGF can shorten the wound healing time, obtain good wound healing rate and total effective rate, promote granulation tissue growth, correct wound microenvironment disorder, and have good safety.
