**** Objective: To analyze the application value of bracket-free clear aligner combined with oral restoration in patients with Angle Class III malocclusion accompanied by tooth defects. Methods: A total of 76 patients with Angle Class III malocclusion accompanied by tooth defects admitted to our hospital from January 2023 to December 2024 were selected as the study subjects. They were randomly divided into an observation group and a control group using the envelope method, with 38 cases in each group. The control group received implant restoration treatment, while the observation group received bracket-free clear aligner combined with implant restoration treatment. The therapeutic effects, masticatory function, and aesthetic outcomes were compared between the two groups. Results: The periodontal excellent rate in the observation group was higher than that in the control group (P < 0.05). After treatment, both groups showed increased bite force and masticatory efficiency, with the observation group exhibiting higher values (P < 0.05). The PES scores in all dimensions of the observation group were higher than those in the control group. In the WES scores, the crown morphology score in the observation group was higher than that in the control group, with a significant difference between the groups (P < 0.05). Conclusion: For patients with Angle Class III malocclusion accompanied by tooth defects, the combined treatment of bracket-free clear aligner and oral implant restoration can improve masticatory function and achieve better aesthetic outcomes.
