Objective: To investigate the influence of the modified internal tragus incision approach on the mandibular motor function and aesthetic effect of patients with internal condylar sac fractures. Methods: Sixty patients with intracondylar capsular fractures admitted to our hospital from June 2023 to June 2025 were selected and grouped according to the random number table method. The control group (n=30) was treated with the traditional anterior auricular incision approach, and the observation group (n=30) was treated with the modified intratragus incision approach. The surgical indicators of the two groups were compared. Mandibular motor function after treatment; The aesthetic effect of the face after treatment; The incidence of postoperative complications. Result: The operation time, intraoperative blood loss and hospital stay in the observation group were less than those in the control group (P < 0.05). The postoperative opening degree, forward extension movement distance and lateral movement distance in the observation group were higher than those in the control group (P < 0.05). The excellent and good rates of facial symmetry and incision scar satisfaction in the observation group were higher than those in the control group (P < 0.05). The incidence of postoperative complications in the observation group was lower than that in the control group (P < 0.05). Conclusion: For patients with intrapcondylar capsular fractures, treatment through the modified incision approach within the tragus can enhance their mandibular motor function, improve the aesthetic appearance of the mandible, have a low incidence of postoperative complications, and achieve good results.
